Referral procedure

Write, phone or call in Mon-Fri 9am-3pm. Application form completed by applicant or referral agency. Application form and admissions criteria also on website. Require proof of ID, references and background reports.

Support offered

Staff available Mon-Fri 9am-5pm, emergency on-call system. Compulsory keywork system with weekly meetings. Action plans with agreed outcomes, reviewed bimonthly. Provide support around independent living skills, training and employment.
