Homeless Health Advocate
This role provides practical, flexible health and housing advocacy across Stockport and the surrounding boroughs in Greater Manchester.
The H4 Hospital Project provides discharge support, housing and health advocacy, and ongoing wellbeing support to homeless patients. The project is delivered by the charity H3 that supports people experiencing homelessness within Stockport to build independence and wellbeing.
The key function of this role is to liaise with relevant parties to identify homeless patients in the hospital and work together with the Homeless Hospital Advocate to arrange suitable accommodation and support for on-going health needs. This will include assisting patients to access on-going and appropriate primary and secondary health care, and supporting them to improve their wellbeing in a wide variety of ways.
The worker will also engage with rough sleepers and people in temporary accommodation to provide advocacy and support to attend GP, Dental, Outpatient appointments
Delivery of deliver healthcare advice, education and support planning to customers living in temporary accommodation, focussed on preventing repeat admissions to hospital is a key part of this role.
Close working with hospital staff is essential to continue to build relationships, raise awareness of and provide training on homelessness and associated issues, helping to embed a culture of early recognition, notification and appropriate support.
The worker will represent this project at an operational and strategic level to a wide variety of stakeholders.
