Working effectively with people experiencing homelessness often means working with multiple service providers. However it can be challenging to bring people together in a way that centres around the individual and enables all parties to work collaboratively.
Multi-disciplinary team (MDT) meetings bring together multiple agencies to jointly assess needs, make decisions and agree actions. Many areas have developed innovative models of MDT that use relational approaches and centre around the wishes of the person themselves.
The forum will showcase two different models of MDT and provide a space for participants to reflect on their own practice in small group discussion with other attendees.
