How to get a referral

Referral agencies

Accept referrals from Housing Solutions Team only.

Referral procedure

Referral form to be completed. Interview to assess support needs. Require supporting information from referral agency and any other agencies working with the customer. https://www.ccp.org.uk/hereford

Support offered

Support staff on-site daily 7.30am - 9.30pm, waking night cover. Support sessions with identified goals. Staff provide support around independent living skills. Signposting and liaison with external agencies as required.
