Referral procedure
All of our referrals come from the K&C 'Single Homeless Team'. Referrals come from either individuals approaching the council to make a homeless application or their support network (Social Services, Probation etc) refers to the SHT. Interviews can take place 7 days a week between the hours of 10 - 20.00. Referrals will need to bring ID (Passport, driving licence, birth cert).

1 x manager, 1 x Team Leader, 3 x Support Workers, 3 x Support Assistants (two are part time and split a full time role between them), 1 x Trainee. The team work on a rolling rota from Monday-Sunday. Shifts are from 07.00 - 15.00 & 14.00 - 22.00. From 21.30 - 07.30 the service is staffed by night concierge workers. Evolve has an on-call manager for out of hours support. Each support worker is expected to complete at least one key work session per week. Customers and Support workers will create a support plan and this will be the basis for ongoing support. Issues covered could include the following: mental/physical health, aspirations, move-on, healthy relationships, budgeting, self-care etc
