Referral procedure

Referral to be send to the relevant email address at any time; Only accept referrals from London Borough of Merton using a MYSHF application form. Interviews are booked within 1 week of referral received;

Support offered

Team: 5 Support Workers; 1 Team leader; 1 Team manager. Team Hours: 9am to 5pm; Out of Hours Service from 5pm to 9am. Key worker Session every month: - Develop life Skills further; - Advise, advocacy and liaison; - Benefit claims and financial guidance; - Sign posting to other services; - Move on to independence living (normally via private rented).
