How to get a referral

Referral agencies

Accept self referrals and referrals from any agency.

Referral procedure

Phone, call in Mon-Fri 10am-10pm or write. Application form completed by prospective resident and agency prior to interview. Require proof of ID and references.

Support offered

1 Senior support Worker, 2 Full-Time Support Worker, 1 Part-Time Support Worker, 1 Student Social worker, 3 Night supervisors. Staffed 7 days per week 9am-6:45am. key worker system, with meetings at least weekly. Action plans with agreed outcomes, reviewed monthly-6 monthly. Provide advice and support around independent living skills, communication, benefits, education, training and employment and help with accessing relevant agencies.
