        How to get a referral
        
            Referral agencies
            Referrals are received directly from Chelmsford City council.
        
        
            Referral procedure
            Chelmsford council will complete a referral form. An interview is then booked and undertaken before a decision is made. Require proof of ID.

            Support offered
            2 full time staff available Monday – Friday between the hours of 8am – 7pm. Care call out of hours system in place for outside of the working hours. Key work system with meetings held weekly. Action plans with agreed outcomes which are reviewed as necessary, at least quarterly. Staff offer help with independent living skills including budgeting, accessing employment and liaising with benefit, employment and healthcare agencies.
