            Who do we help?
            Single vulnerable people over 18 years, with multiple and complex needs; street homeless, alcohol and substance misuse addictions, offending behaviour, mental health and learning disability. Length of stay up to 12 months. Working in partnership and signposting to specialist support services.         How to get a referral
        
            Referral agencies
            All referrals to be forwarded to Gloucestershire County Council Supporting People Team.
        
        
            Referral procedure
            Complete and submit a START application referral form and forward completed referral form to the Supporting People Commissioning team at Gloucestershire County Council. Referrals normally completed by P3 tel: 08081 786003, Homeless Team at Gloucester City Council 01452 396396 or Probation. All forms must be signed and dated by the applicant and immediately sent to the SP Commissioner at the County Council for review before they are referred on to the appropriate supported housing provider.

            Support offered
            On site staff team.  24/7 rota for support service with 6 Support Workers, 4 Project Assistants, 4 Night Workers and a team of Relief Workers. Out of hours rota for additional management support. Weekly support meetings with each customer. Riverside works within the Psychologically Informed Environment (PIE) and Trauma Informed Care models.  Full support planning through thorough risk assessments and needs assessments . Action plans with agreed outcomes and goals reviewed every 3 months. Staff provide support around independent living skills, move-on plans and help with access to education, training and employment.
