Groundswell is a homeless health charity consisting of a community of people with lived experience of homelessness working to improve access to healthcare and tackle homelessness-related health inequalities
Groundswell has been working as part of the Westminster Homeless Health Partnership (WHHCP) to deliver support to people experiencing homelessness and health challenges in the borough. As part of that project they developed a series of resources that support both professionals and people experiencing homelessness to have a better understanding of the role of Case Conferencing.
The Multi-disciplinary Case Conference Toolkit provides information in accessible language about the purpose and role of case conferences and how to deliver high quality meetings. It also includes useful templates such as confidentiality agreements, sample invitation emails and case note templates.
