Who is the project for?

Who do we help?

Single people over the age of 18 years, who are experiencing or at high risk of homelessness. Low /medium support. Not a 24-hour service. Will accept those with drug or alcohol issues. Mental health issues. Domestic abuse victims. Those with a current or past non-violent criminal conviction. Operate a floating support service for families and others in the community at risk of homeless.

How to get a referral

Referral agencies

All referrals will come through the North Devon Hub.

Referral procedure

Referral forms are completed by agencies and discussed at Northern Devon Hub. The referral is placed on a waiting list. The service manager will contact the referred person when a vacancy arises. A time is arranged for the prospective service user to visit the accommodation and have an initial assessment meeting with two staff members, this meeting will define if our service is appropriate for the service user and is where they want to be.

Support offered

1 full time Service Manager, 1 part time team leader, 3 part time Housing Support Workers, 1 part time Floating Support Worker. Service provided 9-5 Mon - Fri with emergency on call system out of hours. Each housing support worker operates a. key worker system, with once a week key work meetings. Once every 3 months a needs assessment & individual support plan is completed and then reviewed weekly. Staff offer comprehensive support to enable a service user to move on into independent accommodation. Staff signpost to other agencies for specialist support with drug, alcohol, or mental health issues. Staff refer to GP surgeries for health related issues. Operates as a Places of Change hostel.
