After referral is received, a member of staff will be in contact to organise an assessment.. This will involve some background information, needs and risk assessment

Support offered

1 manager, 1 full time support worker, twice weekly key-work meetings. Needs assessment and individual support plans and Outcome Star. Sign posting to other agencies for specialist support.
