Who do we help?
Young people aged 16-24 who have been accepted as homeless and in priority need by LB Hammersmith & Fulham. Target 50% women, 20% lesbians and gay men, 50% people from black and minority ethnic groups.

How to get a referral

Only accept referrals from LB Hammersmith & Fulham PATHS team.
Referral procedure
Phone Mon-Fri 9am-5pm. Referral form completed by applicant and agency prior to interview.

Support and accommodation
Support offered
5 staff, 24 hour waking cover. Keywork system with weekly meetings. Support plans based on resident's perception of their goals and problems, reviewed every month. Staff provide support around education, employment, emotional needs and interpersonal skills. Referrals to counselling services and other external agencies. Access to a wide range of activities eg sports, art.
