Referral by Single Access Point City (SAP) of York Council. Referral form must be completed by a referrer on behalf of the applicant

Support offered

5 full time staff based off-site visit daily, emergency on-call system. Support plans with agreed outcomes, reviewed quarterly. Support in 5 key areas: staying safe; making a positive contribution; economic wellbeing; education, training & employment; accommodation.
