Who do we help?

Young single people aged 16-35 who are homeless or at risk of homelessness. Accept applications from young ex-offenders who have not offended for at least 3 months. Priority to those with a local connection.

How to get a referral

Referral agencies

Accept self referrals and referrals from any agency.

Referral procedure

Complete a Supporting People referral form. please email to our gateway team on GatewayDurham@homegroup.org.uk Staff will contact the referring agent to arrange an interview after which they carry out background checks to enable them to complete a full risk/support assessment. Applicant and referring agency will be informed of decision.

Support offered

4 full time staff 1 part time staff. Keywork system (4 residents per worker). key workers are in constant contact with residents. Support plans with agreed outcomes, reviewed at least every 4 weeks. External agencies involved in support plan as appropriate. Support aims to encourage independent living and may vary in intensity according to individual needs.
