How to get a referral

Referral agencies

Accept self referrals and referrals from any agency. Many referrals come from Single Homeless Action Initiative in Derwentside.

Referral procedure

All applications via Gateway - forms are emailed to relevant services. Interview. Require proof of identity, and relevant background information for risk and needs assessments.

Support and accommodation


Support offered

5 full time staff, available 7.30am-10pm. Concierge cover from 21.45pm-7.45am. Linkworker system (4 customers per linkworker) with meetings held every 1 week or as required. Action plans with agreed outcomes, reviewed quarterly. Staff offer support around independent living skills including behaviour management, accessing education and training and liasing with specialist welfare agencies.
