How to get a referral

Referral agencies

Referrals accepted from the Community Mental Health Team and Social Services. Client must have a Care Co.

Referral procedure

Referral form filled in by Care Co. Needs to include Risk Assessment and Care Plan and any other supporting documents. Once all paperwork received, project staff will complete an assessment with the client and this will then be taken to allocations. Care Co will be informed of decision who will speak to the client.

Support offered

4 full time and 4 part time staff on-site Mon-Sun 8.30am-5.30pm, sleep-in cover 5.00pm-9am . Key work system (2-3 residents per worker), weekly key work meetings. Action plans with agreed outcomes, reviewed quarterly. Staff provide help with daily living skills, finance, budgeting, and access to paid and voluntary work, education and training. Involve agencies relevant to support needed.

3 Flats. 2 flats have 4 bedrooms with shared kitchen, bathroom and living room. 1 flat with 3 bedrooms with shared bathroom, kitchen and living room. 2 Rooms downstairs, each with shower room and a shared kitchen area.
