How to get a referral

Referral agencies

Accept self referrals and referrals from advice agencies, social services. LA’s

Referral procedure

Phone daily 8am-9pm for electronic Application form. Assessment conducted within 14 days, decision after a further 7 days. Background checks made with agencies which have previously worked with the person. Require proof of ID - NI card/passport. May require medical history and social reports if appropriate.

Support offered

3 full time staff, sleep-in cover). Compulsory keywork system with weekly meetings. Support plans with agreed outcomes, reviewed 3 monthly. Staff provide support around independent living skills, personal care, access to education and training.
