How to get a referral

Referral agencies

Accept referrals from any agency but these need to go direct to South Lakeland Council to review first of all. Will accept referrals direct from prison

Referral procedure

Referral form needs completed, all forms must be signed by a professional and sent to South Lakeland District Council to review then they send to Home Group and we carry out background check, Interview, Risk assessment. Require ID.

Support offered

3 full time support worker staff, 1 full time support coordinator based on site Mon - Sun 7.55am-8pm working on shift rota, 1 full time senior client service manager. 2 full time housing concierge based on site Mon - Sun 7.55pm - 8.am working off shift rota so service is staffed 24 hours. Operate a key work system with compulsory support meetings each week. Support plans with agreed outcomes, reviewed regularly. Staff offer general emotional support and signposting.
