How to get a referral

Referral agencies

Accept referrals from any agency. Prioritise South Norfolk District Council applicants.

Referral procedure

Phone, call in daily 24 hours or write. Application form completed prior to interview. Require proof of ID - NI card/passport - and further background reports as necessary.

Support offered

24-hour cover. key worker system with weekly meetings. Support plans with agreed outcomes, reviewed every 3 months. Staff provide support with daily living skills, family issues, employment, education and training, signposting to specialist agencies.
