How to get a referral
Referral agencies
Accept self referrals and referrals from any agency, including Probation service with a multi agency approach. All referrals via HPP at Young Devon Exeter (YES Centre)
Referral procedure
Must complete a combined Eastern and Southern Hub Assessment Form. Accommodation is offered following assessment and interview.

Support offered
10 Staff, 24 hour waking cover. Compulsory engagement system, with weekly key-worker meetings. Staff offer life skills support, including cleaning, budgeting, shopping, health management, relationships and cooking. Also education, employment addiction management. We liaise with other local agencies and support providers to ensure that support is available during the planning and after move-on.
