How to get a referral
Accept self referrals and referrals from any agency. Can take referrals from prison depending on level of risk.
Phone Mon-Fri 9am-5pm or email email@example.com to obtain an Application form. Each application is assessed on the information provided by the referring agency. Subject to meeting the access criteria and the availability of accommodation applicants are invited to attend for a comprehensive Need and Risk assessment. The decision to offer accommodation is based on the outcome of that assessment and back ground checks.

Support and accommodation
High Street South is managed by Midland heart and provides low risk low need supported accommodation for up to 15 customers between the ages of 18 to 60. The project is staffed 24 hours, 7 days a week. Applicants must either be homeless or at risk of homelessness and require housing related support to enable them to maintain independent living.
