How to get a referral

Referral agencies

All referrals come via Suffolk Co-ordination Service.

Referral procedure

HRS portal, retrieve application. Interview. Require proof of ID and references. Will require background reports where necessary.

Support offered

14 staff full and part time, available daily 8.30 am-9pm, waking night cover (emergency on-call system if staff are off site). Keywork system (a number of residents per key worker) with monthly meetings. Action plans with agreed outcomes, reviewed monthly. Staff offer help with independent living skills, budgeting, emotional well being, safety and security, employment and training, and liaison with external agencies.
