How to get a referral

Referral agencies

Accept referrals via the Single Access Point from any professional agency. Most referrals come from York’s resettlement pathway from partner agencies.

Referral procedure

Agency referrals only via single access point. Application forms from SAP@york.gov.uk or the website. Interview. Risk and needs assessment. Require proof of ID.

Support and accommodation


Support offered

14 full time and 10 relief staff, 24 hour waking cover. Keywork system (5-6 residents per worker), regular house meetings. Action plans with agreed outcomes, reviewed at least every 3 months. Staff co-ordinate daily resettlement activities for residents through keywork, groupwork, multi-agency working and other activities. Resettlement programme
