Single homeless people with a history of rough sleeping who have high & complex support needs. Applicants referred via HOST must have local connection.

How to get a referral

Referral agencies

Accept referrals from LB Tower Hamlets HOST team and LB Tower Hamlets Through Care Service only.

Referral procedure

Referral form and assessment completed by HOST. Interview and assessment by Tower Hamlets North. May need additional information relating to support needs.

Support offered

4 staff available daily 9am-9pm, emergency on-call system. Key work system with weekly meetings to discuss support plan, including independent living skills and support for move-on. Quarterly reviews. Staff offer advice and information on training, education and career opportunities and independent living skills, and are available outside key work sessions if required.
