Single homeless people with mental health problems. Must be under Care Programme Approach and registered homeless. Priority given to people with a local connection.

No automatic exclusions. Applications considered on a case by case basis.

How to get a referral

Referral agencies

All referral must come through Family Mosaic referral panel.

Referral procedure

Referral forms to be obtained by Family Mosaic call on 01702 445 870.

Support and accommodation


Support offered

2 full time and 1 part time staff on-site Mon-Fri 9.30am-4.30pm. On call service provided out of working hours. Keywork system, weekly keywork meetings. Action plans with agreed outcomes, reviewed monthly. Staff provide support around independent living skills, and access to employment or training. Involvement of CPN, Social Worker, Consultant or other agencies as necessary.
