How to get a referral

Referral agencies

Only accept agency referrals from LB Lambeth Support Needs Assessment and Placement (SNAP) Team. If they cannot provide referrals, welcome referrals from other agencies. Referrals from other agencies including those from prison must go via SNAP Team.

Our only referral system is through the Lambeth MH Pathway.

Acre Lane provides 24hour supported housing. Support is offered to customers around Medication, Tenancy Sustainment, Developing Independent Living Skills, Welfare Benefit entitlement and Education, Training and Employment.
