Young single homeless people at risk aged 16-21 with connections to Lancashire County Council Social Care. Priority given to applicants with a local connection to Hyndburn Borough Council area.

How to get a referral

Referral agencies

Accept self referrals and referrals from any agency.

Referral procedure

Require proof of identity if possible. Applicant must attend an informal meeting at Crossroads. At the meeting the young person will be given a tour of the centre to see what support we can offer them, if both parties are happy a risk assessment will be completed. Staff will then seek relevant background information from partner agencies to ensure meaningful support can be offered.

4 full time support staff on-site. 24 hours cover by support workers and by Night Concierge who also completes a wake up service.. key worker allocated to each resident. Issues covered are:- Support in claiming benefits/Universal Credit Support with debt management Support in becoming more independent Support on improving their health and well-being Ensure that the young person is given the support needed to maintain full time education.
