Referral procedure

Phone Mon-Fri 9am-5pm or download application form from website (post back to address above rather than that given on form). Background information checked prior to interview. Needs and risk assessments.

Support and accommodation

Support offered

Staff are based on-site Mon-Fri 8.30am-4.30pm. Each resident receives about 4 hours support per week. Support plans cover maintaining a tenancy, independent living skills and accessing other services. Use Homeless Outcomes Star to identify support need and achieve objectives.
