Services offered
Charity working with the NHS to set up and support specialist in-reach homeless health teams. Consisting of clinicians, housing workers, and in some cases occupational therapists and mental health practitioners, Pathway Teams help patients to access the accommodation, care and support they need to recover, and get life onto a better pathway after their stay in hospital.

Headquartered at UCLH, with hospital in-reach teams covering Hull Royal Infirmary, Leeds City Hospitals, University Hospitals Plymouth, St George's Hospital London, Homerton University Hospital London, St Mary's Hospital / Charing Cross Hospital / Hammersmith Hospital London, Ealing Park / Northwick Park Hospitals London.
Who is the project for?
Who do we help?
People experiencing homelessness and rough sleepers who are admitted to, or attend A&E departments, in specific hospitals. In the cases of some Teams, specific primary care services may act as the point of referral.
