How to get a referral

Referral agencies

Accept self referrals and referrals from any agency via the Vulnerable Adults Single Point of Access run by Midland Heart.

Referral procedure

Phone the Vulnerable Adults SPA Mon-Fri 9am-5pm who will assess whether referral into supported housing is appropriate and if so identify whether there is an appropriate vacancy in this or in other projects in Birmingham.

Support and accommodation

Support offered

6 full time and 3 part time staff, 24 hour waking cover. Keywork system (2-3 residents per worker), compulsory regular keywork meetings. Develop action plans for each resident. Offer advice and support with independent living skills, budgeting, training, education, employment options and health or personal issues. Must attend residents meetings.
