Single homeless people with low to medium support needs and local connection to Merton (80% bed spaces), the rest are unsupported. Must be committed to finding long term employment, education or training.

Would need to be risk assessed and would depend on support and /or care plan in place.

How to get a referral

Referral agencies

Only accept referrals from Merton Housing Advice, HPU and nomination panel for single homeless referrals for Merton. Accept referrals from No Second Night Out and Merton Housing Advice for no support bedspaces.

Phone LB Merton Housing Advice Team - 020 8545 3714 or 3715 regarding homeless people with support needs referrals. Application form. Interview.

Support offered

17 staff, 24 hour cover, night porter, emergency on-call system. Keywork system with weekly meetings. Personal development plan identifies resident's specific needs, reviewed 6 monthly. Outcomes Star action plan completed within 1st month. Residents must participate in a life skills programme and their personal development plan. Social activities programme. Monthly residents' meetings. Specialist team (CAT) to Supports with Education, Training and Volunteering
