Who do we help?
Young people aged between 16-25 who have been identified as being at risk of homelessness and in need of supported accommodation.
Who we cannot help
Decisions regarding which clients cannot be accepted are made on an individual basis – we would consider their risk assessment and compatibility with other clients already accommodated

How to get a referral
Referral agencies
Accept self referrals and referrals from any agency.
Referral procedure
Referral via local authority homeless prevention panels, children’s services, NHS, other professional agencies and self-referral. Interview with young person and supporting adult if requested by young person. Full risk assessment and all relevant background information is required.

Support offered
Key-work system with weekly key-work meetings. Action plans with agreed outcomes, reviewed every 3 months. Support is provided during the day-time based on individual need and residents can access day and night support for staff working from a project in Ivybridge. On-call member of staff available 24 hours a day, 7 days a week.
