Case management describes how organisations offer support, including processes (referral forms, assessments, support plans etc), how information is recorded (e.g. In-Form), and the overall organisational ethos.
By describing and understanding different approaches to case management, we aim to help organisations plan, evaluate and develop their ways of working in order to improve the experiences of people accessing their services.
This report will be of interest to anyone reviewing their organisational approach, or anyone seeking to understand the homelessness sector in England.
